3. What personal information do we collect about:
1. Patients
As an authorised provider of medical services, we need to collect certain personal information about our patients, including financial information, which is essential to enable us to provide an effective service of care and support. This information is held in individual records (manual and electronic) and other recording systems, all of which are subject to strict security and authorised access policies. Personal information that becomes inactive, for example following enquiries or from prospective patients who do not join the service, is also kept securely for as long as it is needed, before being securely disposed of.
2. Employees
The service applies a secure recruitment policy to comply with regulations under which all personal information obtained, including CVs and references, is, like patient information, kept securely, retained and disposed of in accordance with data protection requirements. All employees are aware of their right to access any information concerning them.
3. Third parties
All personal information obtained about other people associated with the provision of the service, including contractors, visitors and so on, will be protected in the same way as information about patients and employees.
4. How we collect information
Most personal information about patients, employees and third parties is collected directly from them or by completing forms, mainly manually but also electronically for certain purposes, for example when contacting the clinic through its website.
As regards users of the services, we may continue to make use of the information provided in enquiry and referral forms and, for example, during consultations with clinicians, which feed into treatment plans.
As regards employees, personal information is obtained directly and with their consent by means such as references, testimonials and criminal record checks. When recruiting staff, we ask candidates for their explicit consent to obtain all the information necessary for our decision to employ them.
All personal information obtained to meet our regulatory requirements will always be handled in accordance with our policies on explicit consent, data protection and confidentiality.
Our website and databases are regularly checked by experts to make sure they meet all privacy standards and comply with our general security and data protection policies.

We only share the personal information of patients, employees and others with their consent, on a “need to know” basis, observing strict protocols in doing so. Likewise, we will not disclose information about our employees without their explicit agreement, for example to provide references.
The only exceptions to this general rule are cases where we are required by law to provide information, for example to assist a criminal investigation. Even where we seek to inform the local authority of a safeguarding matter or an incident that requires us to notify it, we will only do so with the consent of the person concerned or by ensuring that the information provided is treated confidentially.
Where we provide information for statistical purposes, it is aggregated and provided anonymously, so that its use poses no risk to privacy.
8. How can personal information held by the care provider be accessed?
Procedures are in place to enable any member of staff, employee or third party whose personal information is held by us and could be processed in any way to have access to that information on request. The right of access covers both the information and any use we may have made of it.

Strict protocols are in place to determine how long information is kept by the organisation, in accordance with applicable legislation and regulations.
10. How we keep our privacy policies up to date
The staff designated to oversee and process personal information within our organisation are responsible for continuously assessing all privacy risks and for carrying out a full review of our data protection policies, procedures and protocols at least once a year.
